    Hi Dr Veer, I'm due to get a diathermy turbinate reduction, is this more risky than radio frequency and how can I find a surgeon who uses the exact procedure in your video? Also you say to stop any sprays a month before surgery but I feel extremely unwell if I don't use Dymista before sleeping. My nose blocks up within a day without it.

    Just had mine installed a week ago by Dr Yu here in Atlanta GA. There is a qualification protocol and That includes at least one flunk on cpap ( I had 2), acceptable BMI, mine was the lowest he had ever seen which usually sleep apnea patients have excessive BMI so must lose weight. Third is a test operation where you are put to sleep and your tongue position is checked to see if it is slippong back. Mine was textbook and combined with the low pressure of 4 I was most comfortable with on cpap ( that's the lowest possible Dr Yu gave me an upgraded 90% chance of success instead of the normal 80%. He does about 10 per month. 5 days post op appt he had never seen such little swelling but then again I'm so skinny. In fact the device is plainly evident on my upper right chest and the wire as well as it moves up the neck. He said this would happen and I'm the first patient he's done where this occurred. I'm really skinny. 69 inches 116 lbs. 71 years young. I told him he could put it on top of my head l wouldn't care. Very painful, I have not taken pain meds for 25 years but the Percocet works great although l have used as little as possible because I hate constipation which cleared up immediately. Turn on will be 33 days after the procedure. The hospital literature quoted 77 thousand dollars of which Medicare covered 99.999%. I was stunned but happy. I can't wait for turn on day. An Inspire rep will be there as well as Dr Yu. Best doctor I have ever had, very young but extremely accomplished and personable and explains everything multiple times. Did not take long maybe 90 minutes.

    i highly appreciate the videos you are putting out, but its very surprising that scienctists with all their physics and biotech etc have not found a way to solve a crucial problem of oxygen not properly reachin brain and heart during sleep when they need it most to repair the body in 2024. i suspect a lack of profit. i hope good people like you went one step further and came up with devices and processes to help sleep apnea patients. i am at my wits end in india....looking towards going under anesthesia where a weakened person like me could die doing dise. it looks like the untreated sleep apnea may cause the very procedure to assess it to kill me...wish sleep apnea patients had better help.....would seem like in the year 2024, we would have develope technology to keep airway open for 8 hours in night. seems like a very simple physics problem to be honest....thats not being done coz theres no profit for pharma in it.

      i know its a very crude question...but cant we insert something in "there" to forcibly keep the airway open. some severe patients may prefer irritation over a cutoff of oxygen to brain. im guessing gag reflex and others come into play. very crude idea i know ..but maybe a starting place to work in. is physically keeping those areas open technologically that hard?

    Hi, Mr veer, I’m Julia Andrew and I’m due to have a DISE soon with you and I’ve already spoken with you in feb on the phone. I’m due to have a hospital sleep study in hospital on my birthday actually in august. Anyway I wanted to share with you my experiences I have with the noise I hear in my ears. I’ve worked out myself from doing internet research that I don’t have tinnitus like I thought I had. I do hear thunder and a loud roaring rumbling noise in both my ears. At first I thought it was tinnitus, but realised I could stop and start the noise in my ear myself!! I believe I can voluntary contract on demand, the tensor tympani muscle, resulting in the loud thundering roaring noise I hear. I can definitely contract the muscle on demand, however when I’m having a flare up or crash of ME/CFS i seem to be hearing the roaring fairly constantly for an hour or two. Usually sleep if I can manage to get to sleep helps. I still think I’m controlling the tensor tympani myself but because I’m utterly fatigued it’s easier to contract and maybe I’m not as aware I’m contracting the muscle!!! I’ve heard it’s very rare to be able to contract the tensor tympani muscle. Wonder if it has to do with hEDS, ME/CFS, both of which I have, along with many other medical conditions. I’m looking forward to meeting you in person and having tests done to find out what’s going on with my sleep apnea. My local sleep clinic told me I had obstructive sleep apnea, and put me on CPAP, they are now telling me from cpap results, that I have mostly central sleep apnea episodes and a few obstructive??? This is new information then when I last spoke with you and I’m hoping the tests will find out what’s going on, my local sleep clinic are a bit baffled too as they are not sure what’s going on and wonder if the cpap is causing the central events!!! So they are eager to find out the results of the tests im having in your care! See you soon ! I would be interested to hear what you think of me being able to voluntary contract the tensor tympani muscle?

      agreed - it doesn't make sense - glosso / lingual. weight loss helps a bit (30% or so), but those drugs seem not to work well with OSA patients.

    Hearing my pulse in my ear (Pulsatile tinnitus) started soon after a cold and my ear getting blocked. Exactly as you mentioned. When I lost hearing in my right ear, I was alone in my home and panicked as I was desperately trying to find a sound I can latch onto to let me know that I can still hear ok. I was also getting palpitations and hearing my heart beat was some kind of security blanket perhaps. I think tinnitus for me is due to food intolerances/allergies. I stopped all caffeine sources ie chocolate, coffee, black/green/mate tea even decaf and pulsatile/ringing tinnitus went away within days. Hearing is back to normal too. Another trigger is cinnamon. White chocolate was also a trigger even though it does not contain cocoa powder/caffeine. I think i’m intolerant to the entire cocoa bean, as white chocolate is made with cocoa butter. Caffeine might not be the culprit for others, it might be down to each person finding out their own triggers.

    The stats are not true. Far more than 30 percent stop using CPAP . CPAP is a torture device from a1 5 month experience. I was surprised I could tolerate mouth taping. You can also put a little slit between the tape if you're that worried about it. You can also check out a product that's like a dental dam out of thick silicone called Somno seal. You put it in front of your teeth and it feels less like you're a hostage, but there's still at least a week or two of adjustment and there's more drool. I definitely would be interested in your opinion on the safety of the somno seal. Seems like I can just blow it out if it came to that. It seems to stay in my mouth all night and normally I spit out mouth guards. It does work to make a seal even in your mouth parts slightly. And you can still put a small piece of tape on the lips if one is worried about it falling out. I don't know if you would think differently about it compared to taping or if you have the same concerns. I asked my sleep doctor about my O2 levels since in my watch pad at home sleep study, which I took two of in the course of 2 weeks, showed that I had oxygen below 90 for a significant amount of time. Time. Again, he didn't care what my O2 levels are if my ahi was below 5. I had tried a full face mask and find that more suffocating feeling than the nasal plus as my face relaxes the seal would break more than using nasal cushion. I moved to a bleep system but wanted to get to very low to no leaks to know the most accurate number of pressure I should be on. The bleep system has its own drawbacks but I reduced the leaks significantly where the machine is registering it as 0 L But without taping, without a guard sometimes there is spikes. I don't know what you guys are saying half the time. My experience can be totally different than others. For example, if I try to talk with CPAP in my nose, it's nearly impossible. If I open my mouth too far, it's definitely so uncomfortable that the air is just not rushing out of your mouth like your breathing out of your mouth, if you have CPAP on and your mouth opens, it's a choking feeling. It's even hard to cough. You have to practice but here's the problem. The machine is always on recording you. So if you try to practice it's going to record it as part of your session for that day. And it's going to say you're having obstructions and stuff. These machines are a joke. I mean I woke up. My numbers were fairly good but it was early and it seemed that I had somehow took the device off 4 hours in. Since it was still early I wanted to put it back on and only an hour and a half, of which I was hardly asleep. Now my numbers were double on my ahi because it misrecords normal breathing. But now, because I was trying to better myself from feeling tired all day, I'm branded as someone with obstructive apnea.

      I'm pretty sure that manchester are setting up a similar system for the implant. I am not sure which hospital, but i doubt it will be hard to find out. they did an implant this week as well.

    aren't the problems with this : 1. It is visible under the skin 2. It is reactive so you already can are having at least an hypopnea before it kicks in. In other words, you've already had the events start before it triggers. 3. The wire is on one side so the tongue can twist. My primary doctor doesn't recommend it because he has apnea and his friend does as well . his friend had it implanted and had loss of control of speech when it clicked on. I am Not exactly sure why he was talking as he should have been sleeping but the point was with my doctor was very leery of it And said his friend had a loss of control until it stopped pulsing. To my surprise though, my sleep doctor, who only is concerned about the numbers on the machine, said I qualify for it. it almost seems like the inspire people are in their pocket because the doctors are eager to push it and insurance seems to cover it. He doesn't believe surgeries work. Other than CPAP devices, The only thing he was open to was losing weight or this device because insurance covers it but not things like surgeries. My sleep doctor also says one of his friends is a doctor at Mount Sinai who does plenty of surgeries for apnea and that they just don't work Or work for long for the majority of people, In his opinion. It's hard when you have a doctor against certain things, they won't even let you speak to someone else or have colleagues that find that surgeries don't work either. When I speak to my sleep doctor it's almost like he's a primary doctor where they just do the minimal or what insurance will cover rather than what's the right treatment. I only started a month ago 's so my follow-up appointment. I asked about all sorts of things like a dise study or a titration study to make sure my pressures were correct and to find out what type of obstruction I have. He wasn't sure insurance covered it and didn't think it was important because getting the numbers on the machine down is the same type of indicator that something's working or not. But did I really need a sleep doctor then? I could have bought the machine on my own and played around with the numbers and then any wrong numbers. People would blame me. But that's all they're doing. They lowered the pressure because of complaints I had and just want to see what happens. They don't know that we know how to change the numbers on the machine so he thinks I'm talking to them and they're altering the numbers on the machine. Anyway, it still seems like CPAP not a gold standard but just the standard they get the most results out of the most people. But it still hit or miss. It's torture to get used to it and since I've been using it, don't feel any better on it even when I got my numbers below. Five per hour versus the $40 per hour I was having. That's another joke about CPAP. The numbers are mathematical, wrong and divided over the entire night. Most people in the business will tell you they have no idea why a machine marks certain events. It's just not smart enough to realize the difference between certain occurrences, but that affects you when your doctor is only monitoring the summary numbers, not the actual waveforms. In the USA, many are just saying that doctors aren't paid enough through insurance to spend the time to analyze the data. So they only look at the some report that is sent through the internet to them from the machine. It would be nice Not to have to deal with taping, mouth guards, mask fit issues or applying a tape and a bleep system. And be nice not to deal with a hose. All that stuff is great, but this pacemaker device, well might be life-saving for those who can't tolerate anything else and are having major obstructions, I don't think it's the be all and all device. A device like this is interesting but the whole idea of implanting it and then waiting for things to heal and if it doesn't work now you have something implanted in you that you would need to remove as well as eventually you need to change the battery. Implanting something that can be seen by others is not attractive. I just don't think it's going to turn out to be the solution for many of the people being diagnosed with obstructive apnea.

      for clarity it doesn't stick out of the skin - you can't see it. it is on for every breath - not just the blocked breathing.

      I would also like to see the incision scar size And yes please do make more videos about it because I don't understand how it can be on for every breath. You said it gets triggered once It thinks that you stopped breathing and stimulates the nerves that make tongue move. So if the tongue is already back by that time you've already had the obstruction in my opinion. So yes please do clarify and if you can cite sources that would be great.

    I am a non-dipper with increased diurnal diastolic pressure. It took me almost two years to have my GP send me to the pain clinic. I did my first 1-night test at home (of coiurse without EEG) and I am still waiting the results for 6 weeks now. I am hopeless and I wonder if I should do a private consulation for an in depth assessment.

    This is a very interesting development. It is good that doctors are looking at other methods rather than defaulting to the cumbersome CPAP machine every time. Five years of no sleep and counting for me - quality of life low and life expectancy dropping like a stone 😪 Keep these innovations coming - maybe one will work for me 🤞

      I'm with you brother. 6 months and the longest I've slept is for 3 hours ONE TIME. OTher than that, never more than a hour. And can't get back to sleep once I've woken up. Anxiety has taken over my life. Hate it Hate it Hate it.

    If people want to have this instead of the cpap machines it should be their choice and not up to some doctor. They say you can possibly get this if you can not tolerate the CPAP machine. Well, I say screw the machine, I don't want it. I want this so do it.

      If you don't want the cpap - you can get the implant so long as you also fulfill the other criteria.

    I had procedure today but I am miserable as a huge clunk of skin that was reddish came out and it was at least a little smaller than the top of my pinky should I go back tomorrow

    Thank you Mr. Veer for the good news! I still wake up gasping air (feels like I’m holding my breath) when wearing a tongue retaining device. Does that mean I’m not a candidate for Inspire? I had a modified U3P 7 months ago as DISE suggested my long fatty soft palate is another source of obstructions. I believe (and feel) now my soft palate doesn’t contribute too much to my apnea.

      @@VikVeerENTSurgeon Thank you Doc! My soft palate does open a bit, however, as you said in that video I’m more prone to mouth leaking while on CPAP using a nasal mask due to soft palate seal lost. That is why I’m considering Inspire.

      @VikVeerENTSurgeon hi Mr Veer, thank you so much for your advice. I have severe sleep apnea, my heart rate drops to around 32 and once I breathe again then it will shut up to over 120. The problem I have is I can't find any way of dealing with the masks, as I have boney spurs on the spine right up high so the mask hurts when wearing it and I've already had 3 heart attacks. I saw the consultant once a few years ago, never since I do let the people I do see there technician but I'm honestly not getting anywhere and I do know this will kill me, sorry to sound dramatic, I'm just, well I've lost all hope with them, I'm under the sleep lab at Leicester general hospital.     I kinda regret getting my adenoids and tonsils removed (age 24) because now I can’t breathe while swallowing or I end up swallowing a bunch of air or sucking water into my lungs. If I knew that would happen I wouldn’t have gotten them removed. Has this happened to anyone else??
